The Equity in Athletics Disclosure Act (EADA) Survey is an annual report submitted by coeducational institutions that participate in Title IV (the federal student financial aid program) and have an intercollegiate athletic program. The report overviews institutional participation rates and financial data, summarizing the institution's commitment to providing equitable opportunities for students who participate in women and men's intercollegiate athletics teams.

All coeducational institutions of higher education that participate in any Federal student financial aid program (Federal Pell, Federal SEOG, and Federal SSIG Grants; Federal Work Study; and Federal Family Education, Federal Perkins, and William D. Ford Federal Direct Loans) and have intercollegiate athletics programs must provide information concerning their intercollegiate athletics programs under the Equity in Athletics Disclosure Act of 1994, Section 360B on Public Law 103-382.
